General Liability

When an injury occurs, the first priority is taking care of immediate medical needs.  For emergencies call the UL Police Department:  337-482-6447 or 911.

Accident reporting forms: DA2000 or DA3000 should be filed.  Use these forms to report all incidents that resulted in an injury or had the potential to cause an injury to others on University property or at a University activity or event.  (For auto accidents use the Auto Accident Report form.)

The form should be completed by the Departmenal Safety Coordinator (DSC) along with the persons who have been made aware of the incident.

To file a general liability claim at University of Louisiana at Lafayette, you must report the incident to the Office of Environemental, Health & Safety/Risk Management: 337-482-1840 or safety@louisiana.edu.  This would include incidents involving bodily injury damage connected to university premises or operations.  Be prepared to provide:

  • Date, time, and location of the incident.
  • Description of what happened.
  • Names of witnesses or involved parties.
  • Supporting documentation (photos, medical reports, receipts).

Important Considerations:

  • Timeliness matters: Claims should be filed promptly after the incident.
  • Scope of coverage: General liability applies to bodily injury or property damage linked to University of Louisiana at Lafayette operations, not personal issues unrelated to university activities.
  • Documentation is critical: The more evidence you provide, the stronger your claim.

After submission, follow up will may include being contacted by a claims examiner.  This usually happens within 24 hours of claim submission.  Have copies of all documents and correspondence for your quick reference.  Resolution may involve reimbursement, coverage of medical bills, or denial if liability is not established.
